Meniscus Tears: Physical Therapy or Surgery?




Meniscus tears are a common knee injury that frequently occurs during twisting and rotational movements with the foot on the ground. Historically many individuals have been pushed towards a surgical approach for these injuries, but recent evidence in challenging whether surgery is more effective than physical therapy.


In May 2024, Damsted et al1 published a study in the Journal of Orthopedic and Sports Physical Therapy that sought to compare early surgery to exercise therapy and patient education for treatment of traumatic and non-traumatic meniscus tears confirmed by MRI. The authors found no difference in outcomes between surgical and physical therapy groups at 12 months for either traumatic or non-traumatic meniscus tears. They concluded that surgery did not appear superior to a 12 week physical therapy program of exercise and education at improving function, pain, and quality of life at 12 month follow up.


What does this mean for you? Regardless of how your meniscus tear happened, it does not mean you absolutely need surgery! The most important thing is that your healthcare providers listen to your concerns, educate you on your options, and work with you to choose the best path forward that aligns with your preferences and goals.
